How we handle information submitted through this website

We only collect what you choose to send us through the coalition inquiry form and any email or phone follow-up. The categories are:

• Name
• Email address (required unless you provide a phone number)
• Phone number (required unless you provide an email address)
• RCFE organization or facility name
• Role or job title
• Total licensed beds across your organization
• Counties served and number of locations
• Inquiry type (ready to join, request information, or request a meeting)
• Six-month participation tier, if you select one
• Preferred meeting time, if you request a meeting
• Your message and any questions you include

We also record the date of your submission and a pseudonymous hashed network identifier used only for abuse prevention and rate limiting.

We use this information to respond to your inquiry, share coalition materials and policy updates you request, discuss coalition participation, schedule meetings you ask for, and keep a private record that a request was received so nothing is lost.

This form is only for provider and business contact information and coalition-interest information. Do not submit resident or patient names, dates of birth, medical or insurance information, Social Security numbers, financial information, or other confidential records. This form is not designed to receive protected health information and must not be used to submit it. If confidential information is submitted by mistake, email brandon@goldenbearstrategies.com and we will delete it.

A small number of service providers store and transmit the information you submit. Their handling of that information is governed by the applicable service agreements and privacy terms:

• Lovable Cloud (Supabase) — hosts this website and the database, which is private with access restricted to authorized administrators.
• Resend — transmits the notification email that alerts coalition contacts to a new inquiry.
• The recipients' business email provider — receives and stores that notification and any follow-up correspondence.

We do not sell personal information. We do not share personal information for cross-context behavioral advertising. We do not use third-party advertising trackers, advertising cookies, retargeting pixels, or social media tracking pixels on this website.

This site uses only the basic, privacy-conscious first-party analytics provided by our hosting platform. It records aggregate page views and a small number of non-identifying interaction events, such as when the join dialog is opened or an inquiry is submitted. It does not record the contents of any form field, and full form contents are never sent to analytics or written into browser logs.

Because this site does not track visitors across other websites and does not serve advertising, browser Do Not Track and Global Privacy Control signals do not change how the site behaves: there is no cross-site tracking or sale of information to switch off.

• Test submissions and duplicates are marked as such in the database and automatically deleted 30 days after they are received.
• All other inquiry records in the database are automatically deleted 180 days after they are received. This runs as an automatic nightly deletion job. Records in this inquiry database are not kept beyond 180 days because a coalition relationship is active.

Automatic deletion from the database does not delete the notification email copies that were already sent to coalition contacts. Those email copies may remain in Golden Bear Strategies' business email accounts until they are deleted under Golden Bear Strategies' applicable business-email practices, or sooner on request.

The site is served over HTTPS only. Submissions are validated and processed by a server-side endpoint; the public has no ability to read, change, or delete records in the submissions database, and there is no public directory of submissions. Email and database credentials are stored server-side and are never exposed in the browser.

You can ask us what we hold about you, ask us to correct it, or ask us to delete it. Send your request to either Brandon Knapp at brandon@goldenbearstrategies.com or Samantha Corbin at samantha@goldenbearstrategies.com, including the email address or organization you used. We will verify the request by replying to the address on file and act on it promptly. We will not discriminate against you for making a request.

If we make a material change to this notice, we will post the updated notice on this page with a revised effective date. The effective date at the top of this page always reflects the version currently in force.
